Community Advanced Practice Nurses, Inc. is a non-profit clinic providing free physical, mental, and preventive care to individuals and families who are homeless and medically underserved in the metropolitan Atlanta area. We believe that access to healthcare for the medically underserved is fundamental to empowering people and restoring communities.
CAPN provides medical care at 4 shelters and offers homeless case management for individuals with serious mental illness through the Projects for Assistance in Transition from Homelessness (PATH). Our medical care includes physicals, disease testing, health screenings, sexual health care, immunizations, and community referrals. PATH is a federal program that connects individuals to supportive community resources including long-term housing.
